PSYCHIATRIC EVALUATIONS
A comprehensive assessment conducted by a psychiatric professional to diagnose mental health conditions. This includes a thorough review of the patient’s medical history, symptoms, and any previous psychiatric treatments. This assessment helps identify specific issues and conditions, guiding the development of personalized treatment plans. The overall assessment helps to eliminate any outside causes such as potentially medical related concerns.
MEDICATION MANAGEMENT
Psychiatric medication management is an ongoing, dynamic process that requires regular follow-up assessments to ensure that the patient’s treatment remains effective and safe. By regularly monitoring and adjusting the medication regimen, providers help patients achieve better mental health outcomes and improve their quality of life. Follow-up assessments also provide an opportunity for the provider to offer psychological support and counseling, helping the patient cope with their condition and any challenges they may face with their treatment.
PSYCOTHERAPY
Psychotherapy, or “talk therapy,” is an invaluable tool for improving mental health. In our sessions, we collaborate with you to develop comprehensive care strategies that may include various therapeutic approaches and support services. Tailored approaches that incorporate nutrition, sleep, exercise, stress management, and social support. Designed to empower your mental health journey.
MEDICAL FORMS
I fill out certain medical forms for current clients only. Must be a client for at least six (6) months with initial evaluation and two (2) follow-ups performed in the last six (6) months.

- Psych Evals for surgeries and other treatments
- ESA
- Permanent disability
*To fill out medical forms for FMLA and evaluations, patient must be seen at least 3 times, for an initial evaluation and two follow up appointments. For permanent disability paperwork, patient must be seen for 12 months prior to paperwork. The cost is an additional $100 and processing time is up to 5-7 business days not including weekends and holidays.
GENESIGHT TESTING
GeneSight Psychotropic is a pharmacogenomic test (multigene panel) that uses a proprietary algorithm to evaluate pharmacokinetic and pharmacodynamic relationships, resulting in drug recommendations or warnings for certain neuropsychiatric medications. The GeneSight test currently analyzes 57 neuropsychiatric medications and 12 genes. The test has a precision of 100% in detecting the genes themselves.
WEIGHT MANAGEMENT (SEMAGLUTIDE)
At Mindhaven Wellness and Therapy, we like to tackle mental health problems from every angle, including treating the mind, body, and soul. We believe a well-rounded approach is the best way to achieve a healthy balance in life. For this reason, being physically well including getting good sleep, nutrition, and exercise is just as important as taking care of your mental health. Metabolic disturbances and metabolic syndrome are highly prevalent in patients with severe mental disorders. Glucagon-like peptide-1 receptor agonists can be used to treat metabolic syndrome and pharmacological side effects.

Refill Policies
Refills are sent in for the amount to last until your next appointment. Refills will not be filled for cancelled appointments.
